Dr. Sherri Jacobs, ND, CNS and Dr. Shannon Kennedy, MD, Internist join forces to bring you the best of functional and conventional medicine. Both Doctors collaborate and bring their expertise on every aspect of your healthcare. They take a limited number of patients each year.
The HER membership includes:
Initial Comprehensive Assessment with Dr. Kennedy and Dr. Sherri including a detailed health history review and customized lab testing plan.
Physical exam with lab review, that may include: EKG, pulmonary function test, BIA (body fat, muscle mass, bone mass, hydration status, visceral fat and metabolic age), and breast and pelvic exam.
Quarterly visits with MD/ND team to manage medical concerns, continued nutrient assessment and lab review.
Nutrigenomic (nutrition recommendations related to your genetic make-up) assessment and plan with Dr. Jacobs, ND, CNS.
Bio-identical hormone, monitoring and prescribing.
Up to 4 sick visits for unexpected illness with ND for natural solutions and MD prescriptions as a back-up, if needed.

This is a cash-based program. Labs and imaging are not included in the annual fee, but may have the option of being billed through insurance.
